Man Killed in FBI-Involved Shooting During Warrant Operation

The FBI said the shooting occurred during court-authorized activity, and its Inspection Division is reviewing the incident.

DIXMOOR, IL — A 22-year-old man was killed Wednesday morning in a shooting involving an FBI agent during a federal warrant operation in south suburban Dixmoor, authorities said.

The shooting happened about 7 a.m. near 145th Street and Cooper Avenue as federal agents carried out what the FBI described as court-authorized law enforcement activity. The agency said one person died and no FBI personnel were injured. Family members identified the man who was killed as Chadwick Clark Jr.

The FBI has released few details about what led to the shooting or what happened immediately before shots were fired. It was not clear Wednesday afternoon whether Clark was the person named in the federal warrant or why agents had gone to the home. The agency also had not publicly identified the agent involved.

Dixmoor police said the FBI notified the department before agents attempted to serve the warrant. Local officers helped establish a perimeter around the area but did not participate in serving the warrant, according to police. The Cook County Sheriff’s Office also assisted with the response.

Witnesses described hearing federal agents announce themselves at the home before gunfire. Elika Hughes, who was nearby, told ABC7 Chicago that she heard sounds consistent with agents trying to enter the house, followed by a loud noise and agents repeatedly announcing that they were with the FBI and had a search warrant. Other residents reported hearing several gunshots.

Video from the neighborhood showed a large law enforcement response that included FBI personnel, local police and other emergency responders. A SWAT team was seen bringing a person from the home, and responders appeared to perform CPR before the person was placed in an ambulance. Authorities blocked off part of Cooper Avenue as investigators worked at the scene.

Ruth Perkins, who lives on the block, told CBS Chicago that she and her husband heard loud noises early Wednesday and later heard gunfire. She said she saw agents carry a person from the house and emergency personnel provide medical assistance. Her account has not established what happened inside the home before the shooting.

Family members identified the person killed as Clark, 22. CBS Chicago reported that Clark’s mother said he was one of seven children. Authorities had not released an official account explaining whether Clark was armed, whether shots were fired at agents or what circumstances caused the FBI agent to open fire.

The FBI said its Inspection Division is reviewing the shooting under agency policy. The bureau said it takes shootings involving agents or task force members seriously but declined to provide additional details while the matter remains under investigation.

The Dixmoor shooting was the second fatal incident involving an FBI agent in the Chicago area this week. On Monday, the FBI said an off-duty agent fatally shot a man in Chicago’s Pilsen neighborhood after encountering a person who was firing toward a group of people. The circumstances of the two shootings were separate, and authorities have not indicated they were connected.

The investigation in Dixmoor remained active Wednesday, with major questions about the warrant operation and the moments preceding the shooting still unanswered. The FBI had not announced when additional findings from its review would be released.
